GENERAL INFORMATION

Effective December 2006: ONLINE ROSTERS AND REGISTRATION RENEWALS. Providers who are interested in obtaining access to their online rosters or renewing registrants online may obtain further information by contacting the PI/SG Unit at (614) 466-4130 or PISGLR@dps.state.oh.us.

NOTE: Applications requesting a new firearms bearer (FAB) notation postmarked on or after October 24, 2005 are required to have a FBI criminal records check in addition to the BCI&I criminal records check. BCI&I accepts FBI fingerprint cards for PI/SG FAB applications. FBI fingerprint cards can be ordered through BCI&I. Please remember that fees for both criminal records checks must be sent directly to BCI&I.

National WebCheck (NWC) workstations can now be used to process PI/SG FBI criminal records checks. A listing of NWC locations can be found on the Attorney General's Web site (www.ag.state.oh.us), by clicking on the drop-down arrow in box marked "Quick Links," click on the "Web Check" link, then click on the "WebCheck Community" link. Any WebCheck location listed with "(NWC)" written next to the location name has National WebCheck available. Please contact the location directly to determine if PI/SG FBI criminal record checks are available at that location.

REGISTRATION APPLICATION FORMS
(Required by a business to register their employees)

Employee Registration Application and Instructions -- PDF Format (HLS 0012): Required to register new employee no sooner than three days nor later than seven calendar days after the date employee is hired


PRIVATE INVESTIGATOR/SECURITY GUARD LICENSE APPLICATION FORMS
(Required to obtain a company Private Investigator/Security Guard license)

DMA Form for State Issued Licenses - As a result of Senate Bill 9, applicants applying for certain state licenses, including Class A (PI&SG), Class B (PI) and Class C (SG) provider licenses and renewals, are required to fill out Declaration of Material Assistance or DMA forms.
