All four DMA forms have been updated. The
updates include an instructional page as
well
as some small
changes to the vendor form for clarification purposes. OLDER
VERSIONS OF THE FORM MAY STILL BE ACCEPTED AS VALID, however
government entities should begin utilizing the updated forms as
soon as possible.
Completed DMA forms are NOT to be sent
to Ohio
Homeland Security. They are to be submitted to and retained by the
issuing
agency or entity.
The Terrorist Exclusion List has been updated
pursuant to House Bill 231. The revised list is effective as of
July 20, 2006.
